Your plan is the "Standardized Proft Sharing/CODA" plan in the IRS list you reference above.Would appreciate some guidance from those who understand this.

I found the link to the list of "defined contribution plans submitted from October 2, 2017, to May 13, 2024" , which is: https://www.irs.gov/pub/irs-tege/ppa-listdc3.pdf
- 1. I believe my plan is a defined contribution plan (vs. defined benefit plan) because I determine the annual contribution, and the benefit is the result of whatever the market yields when I am ready to take benefits. Is this correct?
Once I selected the link for (what I think is) the proper IRS pdf list, I had to find my plan sponsor.
- 2. The section "from October 2, 2017, to May 13, 2024" is the correct section to select (vs "defined contribution plans submitted from February 1, 2011, to April 2, 2012") because is it the newest. Is this correct?
- 3. On the IRS pdf list above, Fidelity is not listed. However, Per the 2021 Restated Agreement Documents Fidelity distributed and had me sign and keep in my files, the Adoption Agreement Document, section 9. states "Name of Provider: FMR, LLC" Is what the IRS labels as "Plan Sponsor" the same thing as what the Adoption Agreement labels as "Provider"? ie if my Solo 401k is with Fidelity, is my Plan Sponsor FMR, LLC?
- 4. There are 5 FMR LLC plans. I have a Fidelity Self Employed 401k. My 2021 restated agreement says my plan is a "Non Safe Harbor 401(k) Plan." Of the 5 options on the IRS pdf, is that what is listed as a "Standardized Money Purchase" plan type?
Thanks in advance for guidance on the above. Either I've totally missed the mark with the above, or their lack of consistent terminology makes this far more confusing than it needs to be.
- 5. Question 12 of the 2023 IRS 5500 EZ states "If the plan sponsor is an adopter of a pre-approved plan that received a favorable IRS
Opinion Letter, enter the date of the Opinion Letter / / (MM/DD/YYYY) and the Opinion Letter serial number." The IRS pdf provides both a FFN number, and a LSN number that has an accompanying date. Is the LSN number and date corresponding to the plan type identified in item 4 the correct number to enter on the form 5500EZ?

The date and opinion letter number question matches the date and letter number in the IRS list.
Also, if you file on efast.dol.gov (highly recommended) I believe you will need to include an "a" (lowercase) at the end of the opinion letter number.
