There is a big difference between travel insurance, which covers travel costs, and overseas medical/evacuation insurance. We generally self-insure our travel expenses, but not the risk of a serious medical event while overseas.
To be clear, most "travel insurance" policies include both travel costs *and* medical/evacuation coverage.
Some people specifically select a low or very low amount for the "trip costs" because they are primarily - or only - interested in the medical/evacuation coverage.
There is a variety of coverage amounts available for each of these, in the policies offered by a variety of travel insurers.
One insurer that includes only medical coverage but doesn't include any of the trip costs (I *think*; we haven't had this type of coverage) is GeoBlue.
What is very important with any of the travel insurance policies is to understand all of the Terms & Conditions of any policy one is considering, including what "events" are covered and for what limits.
