1099 UPDATE - I did my Ascensus to Fidelity solo 401K transfer back in August the "old" way by terminating myself as an employee. I didn't get any communication about my 1099, but I logged in to the employee portal and saw the 1099-R was ready to download. It shows distribution code G and "0.00" for taxable amount.

It looks just like any other 1099-R I've gotten when I rolled old employer 401Ks into another 401K or IRA. If I'm remembering correctly, our main concern is that it looks like we opened a new solo 401K without waiting 12 months? But I have a clear paper trail to show the IRS that it's the same solo 401K plan.
You're still able to log into the employee portal? Didn't you lose access to that when you were terminated as an employee? Or did you mean the employer portal? And if you did, where in there did you find the 1099-R?
I can still log into both. After I terminated myself to get the rollover check, I removed the termination status. I never lost access to my employee portal. I completely forgot that I did this.
