I had this exact problem with Vanguard, 5 times (going to another custodian/trust).
I filed 2 FINRA complaints and received apology letters. I initiated a case with an attorney and began the process of filing suit against Vanguard.
The final check that mysteriously never got to a large reliable custodian got remailed the 2nd time, but this time via Fedex with tracking after I advised I would file suit within 24 hours.
I then transferred 7 figures in various accounts to their competitor and closed everything.
I'm delighted to never have to deal with these incompetents again.
