This is Vanguard being cheap and minimalist. They are not doing anything more than FINRA or the SEC require in order to keep expenses low. That's the downside of compensating executives for lowering costs even if its exposing their clients to greater risk.
I'm curious, are there any reports of customers actually losing money? I completely agree that I wouldn't want to go through what the victims in the article went through, but there seems to be lots of blame and assumed lack of action going around without an real loss of funds.
