My individual HSA is at Fidelity, easy to open, fund, and use. (You don't need a debit card, just make a withdrawal via a transfer anytime you'd like.)
If it were me, I'd ask Fidelity for advice on the max allowable pro-rated contribution for this year since you're looking to do catch-up contributions this year as well.
