The entities where I am a member of the finance committee that gives out funds from the endowment does not have any forms. First off, one has to follow the wishes of whoever funded a particular endowment pot. Second, the mission statement of the church should give some insights, too. Third, the Finance Committee gives advice to the Executive committee who makes the final decision.
But the requests we get come from entities and people that are used to writing grants to government agencies and major non-profits. Those places put their grant forms on their web sites such as NIH, NSF, American Cancer Society and the like, so you might be able to get something by a web search for entities that are similar to yours. However, the ones I mentioned are multiple pages and multiple dollars. I would guess that you are mostly looking for smaller requests such as "Please donate $500 towards the Thanksgiving Dinner for the Homeless Shelter at 123 E. Main Street, My Town, My State, USA." If you need to create a form for that, then ask for documentation either before and/or after that the money was actually used for the request (l.e. receipts) and not simply pocketed.
