If your Roth IRA is qualified, you do not have to report it. See instructions for Form 8606 Part III.
If the Roth distribution is qualified, then Form 8606 does not apply. But the qualified distribution is still required to be reported on the tax return - on line 4a of the 1040.
That's a good point. For some reason, I thought the original poster was just asking about reporting it on 8606 Part III. I agree with you that the 1099, when entered, will show up on line 4a but not 4b.
