The form should specifically reference the No Surprises Act. The form you are taking about has been around forever and doesn't appear to meet the consent requirements of the new law.

In that case, I take it that one should check for language specifically and adequately referencing the No Surprise Act.

In the event the document is lacking, decline to sign and request instead an updated form.
It's the opposite. You can probably sign anything that doesn't reference the No Susprises Act. The document that does reference that act will likely be the one asking for you to waive your rights under the act.
