I know for a fact BYU doesn't. I haven't called any others, but I suggest you try. I read the website and it almost implied that you had to fill it out, but I kept looking and looking and never saw them say it actually was required. Turned out it wasn't.All the schools in Utah have. I guess check with where you are applying.
