* calling the national service center -- didn't work. I reached a national person who did nothing, said he needed to transfer me to a manager, but actually transferred me to my local unresponsive office
* corporate office -- I found the CEO's email and wrote him today. Hoping this will work.
* ethics hotline -- thanks for this tip. They bury it pretty deep on the website, but google found it. I'm keeping it in my pocket
* small claims -- in my state small claims is only for monetary damages; you can't use small claims to "force" them to close your account or stop harassing you

This is brilliant solution to show a monetary loss for the purposes of small claims court. Much thanks -- if emailing the president produces no fruit, this seems worthwhile to pursue.If I receive another bill/threat from your company, I will be forced to file in small claims court to recoup this $5.43 and any further expenses".
