As noted above, ask for an itemized invoice. In VA, this is required by law to be provided to the patient, free, upon request:

My guess is that most states have a similar law. In VA, complaints about providers who don't comply are sent to the boards which license them.
The medical records can also be requested to (in VA there is a modest charge) to compare against the invoice.
