VTSAX/VTI/.... has a 0.03% ER and is by even those who make this argument largely immune to the front running problem because it has excellent index construction and size enough to make its own moves against front-running.So what you might have presumed was a low cost index fund may just end up giving you what you pay for.
Sometimes there is a free lunch.
This is not true of all indexes or index funds, they need well constructed indexes with processes in place to reduce front running and usually need to hold effectively 100% of the addressable market cap.
