As I am a sole proprietor, I guess I should put: "2E, 2J, 3B, 3D". Right?

2. And then for line 12 regarding the IRS Favorable Opinion Letter: For the date I can only find a letter online with date 12/31/2018 (or 2020 at the end apparently)

The last line in the header (on the right side) is Date: 09/21/2020. That's what I put on Line 12.

And for serial number I see: Q704157a
