- I read multiple incidents of people who didn't notice the new question added to Form 5500-EZ this year (the opinion letter serial number), and submitted their forms without filling it in because they blindly transferred the previous year's values (and the online validator didn't seem to catch this mistake).
Thank you for this. I glossed over this question when completing the 2023 form earlier this year because it did not seem to apply -- we custody our Solo 401(k) plan through E*Trade and used their standard adoption agreements to open both trad and Roth sub-accounts for our two owner-employees. It did not occur to me that E*Trade's plan template was subject to a favorable Opinion Letter that should have been cited in our 5500-EZ filing.

But I just searched the forum for more insight and found this thread from earlier in the year:

So my next question would be whether anyone has any tips/guidance/admonitions about amending a 5500-EZ to address this omission.

I see the DOL/EFAST points people to Q4b in this document but it's not especially enlightening:
